The film is based on a concept for a feature film that Leone started working on shortly after directing his first short film, The 9th Circle. After the release of Terrifier (2016), Leone wanted to bring the heroine back as the protagonist. She would evolve into Sienna, who Leone describes as the “heart and soul” of Terrifier 2.
Art the Clown returns to the timid town of Miles County after being revived by a sinister force. On Halloween night, he targets a teenage girl and her younger brother.

Damien Leone is the writer, director, editor, and producer of the American splatter slasher Terrifier 2, which will be released in 2022. It is the third feature-length film to feature Art the Clown and is a sequel to Terrifier (2016). David Howard Thornton and Samantha Scaffidi, who played Art the Clown and Victoria Heyes in the first movie, return to the role. Other cast members include Lauren LaVera, Elliott Fullam, Sarah Voigt, Kailey Hyman, and Casey Harnett.
A year after the events of the first movie, the story follows Art’s resurrection and pursuit of Sienna Shaw (LaVera) and her younger brother Jonathan (Fullam) on Halloween night. Dark Age Cinema and Fuzz on the Lens Productions are the production companies of Terrifier 2 which is distributed by Bloody Disgusting.
